Why Regulated Einsteins Created This

We created this snapshot so people can choose how their communication preferences are shared. Useful details might include how they receive questions or what others should offer or avoid.

Start here: Record one preference in your own words or usual communication method. Decide whether to add it to the shareable summary and who may receive it.

Use it for appointments, school, work, community settings, new support relationships, or personal self-advocacy. The person controls what is included and who receives it.

Using your files

Choose the printable PDF for handwriting or the fillable PDF for typing. Save a private copy, then close and reopen it to check that your answers were saved. Print on U.S. Letter paper at Actual size or 100% using black ink or grayscale.

Use only useful pages and prompts; fields may be left blank. White pages, clear prompts and writing space carry through both versions.

The shareable summary and its optional name field start blank. Private working answers do not copy automatically into the summary. Enter only the information you choose to share.

Common questions

Can someone help me use it? An invited supporter can help the person record their own words, communication and preferences. The person controls what is included and who receives it.

How do I open the fillable file? Download it and use a PDF reader that supports forms. Keep answers within the visible space and check the saved copy before relying on it.

What do the previews show? Actual pages from the printable PDF: its opening page and two interior pages.
